Bacteriostatic Water vs. Sterile Water: What the Difference Actually Is

Both liquids look identical, both are labeled "water for injection," and both show up constantly in peptide-handling discussions β€” but they are not interchangeable, and the difference comes down to one ingredient. This explainer covers what separates bacteriostatic water from preservative-free sterile water, based on FDA-indexed prescribing information, so a lab knows which diluent fits a given use case.

The one ingredient that changes everything: benzyl alcohol

Per FDA-indexed prescribing information on DailyMed, Bacteriostatic Water for Injection, USP is a sterile, nonpyrogenic preparation of water that contains 0.9% (9 mg/mL) benzyl alcohol added as a bacteriostatic preservative (some formulations are labeled at 1.1%). Benzyl alcohol doesn't sterilize the vial outright β€” "bacteriostatic" means it inhibits further bacterial growth once the seal has been punctured, which is what allows the same vial to tolerate more than one withdrawal.

Sterile Water for Injection, USP, by contrast, contains no preservative at all. It is manufactured sterile and nonpyrogenic, but once the seal is broken there is nothing in the solution controlling microbial growth going forward.

A documented precaution: benzyl alcohol and neonates

FDA-indexed prescribing information carries a specific caution: benzyl alcohol has been associated with toxicity in neonates ("gasping syndrome"), and where water is needed to prepare or dilute a product intended for use in neonates, only preservative-free sterile water is used.
