What is MOTS-c?

MOTS-c is a 16-amino-acid peptide encoded within the mitochondrial genome, specifically within the 12S ribosomal RNA gene. Unlike most cellular proteins, which are transcribed from nuclear DNA and imported into mitochondria, MOTS-c is transcribed directly from mitochondrial DNA and translated within the mitochondrial matrix. This unusual origin gives it a role distinct from other peptides: signaling between the mitochondrial and nuclear genomes to regulate cellular energy metabolism.

Core metabolic mechanism: AMPK and PGC-1α

The primary mechanism by which MOTS-c is studied involves activation of AMP-activated protein kinase (AMPK) and downstream activation of PGC-1α (peroxisome proliferator-activated receptor gamma coactivator 1-alpha). However, on July 23–24, 2026, the FDA's Pharmacy Compounding Advisory Committee voted 8–6 to recommend MOTS-c for inclusion in the 503A bulk-drug-substances list — a non-binding recommendation. The FDA has not yet formally adopted this vote, and the final decision remains pending.
