Retatrutide vs Tirzepatide vs Semaglutide: One, Two, or Three Receptors
These three compounds are constantly discussed together, and the reason is structural: they represent a stepwise ladder of receptor engagement. Semaglutide hits one incretin receptor. Tirzepatide hits two. Retatrutide, still investigational, hits three. Understanding what each added receptor contributes is the clearest way into this literature β and the 2026 Phase 3 data lets us look at whether adding receptors keeps paying off at the same rate.
The three receptors
- GLP-1 (glucagon-like peptide-1) β an incretin hormone receptor. Agonism is associated in the literature with glucose-dependent insulin secretion, slowed gastric emptying, and central appetite signaling.
- GIP (glucose-dependent insulinotropic polypeptide) β the second major incretin receptor. Its contribution alongside GLP-1 is an active research question, with proposed roles in insulin secretion and adipose tissue handling.
- Glucagon receptor β the counterintuitive one. Glucagon raises blood glucose, but glucagon receptor agonism is also associated with increased energy expenditure and hepatic lipid handling. Combining it with GLP-1/GIP agonism is the design bet retatrutide makes.

The TRIUMPH Phase 3 program
Retatrutide's pivotal trials run under the TRIUMPH name. The reported readouts:
- TRIUMPH-1 β all studied doses (4 mg, 9 mg, 12 mg) met primary and key secondary endpoints; participants on the 12 mg dose reported average weight reduction of 70.3 lb, or 28.3%.
- TRIUMPH-4 (December 2025) β participants reported losing up to 28.7% of body weight (average 32.3 kg) at 68 weeks.
- TRIUMPH-2 and TRIUMPH-3 (July 2026) β positive topline results reported, with improvements in both weight and A1C.
The manufacturer has indicated plans to submit for U.S. approval in Q1 2027. Until a regulatory decision, retatrutide remains investigational.
The part worth noticing: diminishing returns
Run the gaps. Semaglutide to tirzepatide β adding the GIP receptor β is roughly a 7β8 percentage point step. Tirzepatide to retatrutide β adding the glucagon receptor β is roughly 6 points. The third receptor adds real effect, but it adds less than the second one did.
That is a meaningful observation for anyone reading this literature. The "more receptors is proportionally better" intuition does not hold cleanly, and it raises the obvious research question about where the curve flattens. It also explains why the next wave of compounds under study β amycretin, pemvidutide, mazdutide, VK2735 β are not simply stacking more receptors, but exploring different combinations and different pharmacokinetics.
Why this comparison dominates search
Roughly 60% of peptide-related search volume in the U.S. concerns weight loss, and the top three peptide search terms are all GLP-1 receptor agonists, together accounting for over 2.2 million monthly searches.
